GENERAL DESCRIPTION OF POSITION:
The Family Health Advocate will onboard patients properly, facilitate enrollment of patients into affordable health insurance coverage, assist patients in linking with community health resources and the sliding fee scale if needed. This position will also engage in in-reach/outreach and customer service activities. The position requires strong communication and self-motivation skills.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
  1. Onboard new patients with an introduction to the benefits of North Country and their medical home.
  2. Attend trainings and become knowledgeable about the health insurance options in Arizona.
  3. Provide health insurance coverage information and application assistance in a fair, accurate, and impartial manner. Provide information in a manner that is culturally and linguistically appropriate to the needs of the population being served.
  4. Responsible for staying well-informed of and communicating information related to all of North Country's services.
  5. Enrolls patients in the sliding fee scale discount program and ensure that all program requirements are met.
  6. Limited travel around communities in the regions to provide outreach/education and enrollment assistance at events as well as placing flyers and promotional materials in identified locations.
  7. Track activities and provide data summaries as requested.
  8. Promote the North Country patient portal and instruct patients how to use this tool.
  9. Provide excellent customer service and utilize marketing materials related to programs, services, medical home and health insurance coverage.
  10. Educate patients about care opportunities related to their age and health insurance plans.
  11. Become knowledgeable about the benefits for different insurance plans and communicate this to patients.
  12. Hold regular Enrollment Workshop Sessions and assist individuals in the completion of applications and other paperwork needed to obtain services.
  13. Promote Supplemental Nutrition Assistance Plan (food stamps) and help to enroll families in the benefit.
  14. Train and assist clinic staff and students about the sliding fee scale, insurance coverage options, community health programs, patient onboarding, community services, medical home, and customer service.
  15. Attend and engage in clinic meetings, organizational patient retention meetings and other committees as needed.
  16. Perform any other related duties as required or assigned.
QUALIFICATIONS:
Graduation from high school or GED and experience or training in a social service, health, or disabilities related field. Must be able to carry out written and oral instructions, deal with variables and determine a course of action. Be able to define problems, collect data, establish facts and draw valid conclusions with accuracy. Have the ability to maintain accurate records both manually and electronically, develop good rapport with clients, community leaders and fellow workers.
