General Description

This is a 20 to 40 hours per week Program Specialist/Health Educator position for the Domestic Violence Medical Advocacy Program for Rural Communities under the Outreach Division of North Country HealthCare in Holbrook, Arizona. The Program Specialist/Health Educator will provide crisis intervention, advocacy and case management for victims of intimate partner violence and their children in the clinic setting. Services include but are not limited to: needs assessments, developing case plans, safety planning, screening and crisis intervention, referrals for community based services, client education and outreach. Additionally, the Program Specialist/Health Educator will provide training to health care providers on the dynamics of intimate partner violence.

Overall Position Responsibilities

  • Conduct assessment of victims' needs for service
  • Assist in development of an individualized case plan
  • Assist victims in the monitoring and updating of the case plan
  • Assist victims in the completion of applications and other paperwork needed to obtain services.
  • Assess the needs and make appropriate referrals to community-based services
  • Assist with safety planning and obtaining orders of protection
  • Accompany victims to appropriate court appearances
  • Provide crisis intervention as needed
  • Maintain accurate and timely documentation of case records
  • Assist in reporting and data collection, as assigned
  • Perform other duties as assigned
  • Research funding opportunities and write grant applications

Minimum Qualifications/Skills and Abilities

  • Bachelor's degree preferred
  • Demonstrate an understanding of dynamics of domestic violence
  • Experience working with victims of domestic violence
  • Experience in crisis intervention and making referrals for services
  • Strong organizational skills
  • Demonstrated community organizing skills
  • Demonstrated cultural competency working with Native American and Latino communities
  • Self-motivated, able to work with minimal direct supervision
  • Strong oral, written and computer skills
  • Bi-lingual (English and Navajo) preferred
  • Ability to work in a team, problem solve as a team/collaborate
  • Ability to advocate for clients to ensure their best interests are met
  • Ability to develop safety plans with clients
  • Ability to case manage
  • Ability to seek out resources for clients (knowledge of who to call for resources)
  • Ability to travel throughout the community
