Description
This is a full-time, Medical Case Manager position for the HIV/AIDS Ryan White Part B Program (RWPB) under the Outreach Division at North Country HealthCare. The Medical Case Manager will work under the supervision of the Program/Project Coordinator, and is part of a team with other Medical Case Managers to coordinate the medical care of patients living with HIV/AIDS on the Navajo Nation, Hopi and Coconino County areas. Coordinating care involves linking clients to services in areas such as dental, primary care, HIV specialty care, behavioral health, and support services. In addition, the Medical Case Manager will help clients navigate different insurance systems and access other non-Ryan White resources and services available to HIV patients. The position requires strong organizational skills and verbal communication skills, along with knowledge and understanding of the Navajo and/or Hopi language, culture, and understanding of the HIV/AIDS population, barriers to care, disease prevention and transmission, and sensitivity to the social stigma that exists for persons living with HIV.Duties
- Work as a Medical Case Manager to determine and maintain eligibility of clients and connect clients to services. Engage in care coordination for optimal patient health outcomes.
- Enhance and develop relationships with all HIV medical providers and HIV/AIDS service organizations and other consortiums/coalitions serving persons living with HIV in the service area.
- Facilitate, organize, and utilize systems for tracking client services and demographic information.
- Help patients navigate through all medical services and procedures including the Ryan White systems.
- Maintain accurate and complete client files, treatment plans and program databases including entering client information into EHR and the CAREWare database.
- Promote North Country's HIV services to organizations and groups and disseminate HIV/AIDS-specific information.
- Aid in the implementation of mental health and growth of client support groups.
- Travel to visit clients and attend relevant meetings and trainings as necessary.
- Participate in planning, implementing, and evaluating the RWPB Program and its educational goals and objectives.
- Participate in community health fairs and perform screenings as required.
Requirements
- Two year college degree or equivalent experience resulting in knowledge of a field related to the job required
- Two years' experience in healthcare or social work
- Bilingual (English/Navajo/Hopi/Apache/Spanish) preferred
- Travel required
