General Description of Position:
As a member of the integrated health care team, the advanced practice pharmacist provides specialized pharmaceutical services to patients and providers to improve drug usage and therapeutic outcomes for North Country HealthCare. Responsibilities also include coordinating with other NCHC pharmacy technicians and pharmacists and participation in provider and pharmacy student's education. The advanced practice pharmacist may practice in the dispensing services area or clinical pharmacy service area. The ratio of dispensing and clinical services to be delineated by Director of Pharmacy at the time of hire.
Essential Duties and Responsibilities:
- Conduct and evaluate medication histories, assess compliance and suggest modifications for optimal drug product selection in order to achieve desired outcomes; instruct patients in correct use of prescribed medications.
- Individualize medication regimens using sound principles and understanding of drug absorption, distribution, metabolism and elimination to establish and improve to optimum patient benefit.
- Prevent, detect and resolve medication-related problems. Make evidence-based, patient-centered drug therapy recommendations as part of the Integrated Care Team in a Patient Centered Medical Home Model.
- Utilize clinical pharmacy skills, including contraindication and duplicate therapy checks, in assessing and formulating recommendations to physicians ensuring quality and economical drug therapy
- Actively participate in the PGY1 Community-based Pharmacy Residency Program through continuous quality improvement, precepting residents and other activities as assigned.

Education And Experience:
- Professional degree (J.D., Ph.D., etc.)
- 5 years related experience and/or training.
Required Certifications, Licenses, Registrations:
- Five years of experience in a clinical setting OR completion of an accredited clinical residency program.
- Current Arizona License or eligibility for license required, and Current CPR certificate
Preferred Certifications, Licenses, and Registrations:
Certified Diabetes Educator (CDE) Residency Board Certified Pharmacotherapy Specialist (BCPS) or Ambulatory Care Pharmacy (BCACP) APHA disease state management certifications (MTM, Diabetes, Immunizations) Demonstrated ability to achieve positive patient outcomes in a similar environment Past experience as IPPE/APPE and residency preceptor
