Overview of Position:

The Population Health Manager is responsible for overseeing the development, implementation, and evaluation of population health programs to improve health outcomes for our community. The Population Health Manager collaborates with internal and external partners to strategically improve performance against quality-based metrics (HEDIS, UDS, PCMH). The position is an integral part of designing and optimizing improvement efforts related to Value Based plan requirements including timely annual wellness visits, chronic disease management and preventive care.

Duties:

  • Plan, manage, direct, and coordinate population health and value-based care activities to achieve organizational goals and optimize performance in value-based contracts and incentive payment models.
  • Effectively communicate population health goals and strategies to health system leaders and other relevant stakeholders.
  • Develop programs to ensure successful implementation of initiatives and processes for monitoring clinical outcomes that align with NCHC mission and goals.
  • Manage patient engagement specialist staff to support value-based care efforts.
  • Oversee Population Health Clinical Program in conjunction with the Quality and Population Health Medical Officer.
  • Collaborate with HIT partners on quality-related data collection and hygiene.
  • Analyze population health data to identify trends and areas for improvement.
  • Collaborate with other members of the healthcare team to identify and address the health needs of the community.
  • Develop and implement interventions to improve population health outcomes through quality improvement projects.
  • Collaborate with finance department on value-based efforts and population health team aspects of the Quality Department budget.
  • Contribute to strategic planning for the Quality Department regarding population health goals.
  • Pursue training and development opportunities and adapt to new technologies.

Qualifications:

  • Bachelor's Degree in Nursing required.
  • Professional clinical healthcare license/certification (RN) required.
  • Master's Degree in healthcare related field such as Public Health Nursing, Health Care Administration, or Clinical Informatics required.
  • Three to four years related experience and/or training in community health, value based care, quality improvement, or related field required.
